Online Student Evaluation SurveyNeed an easy way to collect Student Evaluations for your Distance Learning course? Use the Distance Learning survey tool - QAAL - Question and Answer Library.
We have already created an electronic version of the Student Evaluation survey found in the FHDA Faculty Handbook. You can start with that. If you would prefer to customize that survey instrument to meet your specific course needs, it is easy to modify our existing template or create one of your own.

Creating a Student Evaluation Survey Form

Open the QAAL main page in your browser window.

Create a QAAL account. Your ID is your email address. Enter a password as well. Once you have created an account, you can Login from the main QAAL page, using your email address and password.

You will see a list of pre-built surveys under Open Source Forms. You can choose to use one of these as the basis of your own student evaluation survey form by clicking on link to the survey title. A copy of this survey form will be made and added to your list of forms.
- If you want to create an entirely new survey form, click on the Create Form link on the left of your screen. Compete the requested information and click the Create Form button.
- Now that you have a survey set up, you can add or modify questions in your survey. Click on the link to your form
to move to the questions update page.

To ADD a question, click on the Add Question link on the left of your screen. Fill in the question text, select from the predefined set of answers or compose your own. Then Click on the Create Question button.
- Add questions one at a time using this process, until all your questions have been added.

View your survey. Click on the View this Form link at the top of your screen.

Your QAAL survey can be accessed directly from the Short URL listed at the top of your screen.

Student responses
As students complete and submit their student evaluation surveys, you will receive an anonymous email with the information provided by the student. There is no information about the source of the email, as this is all handled by a program.
Reorder questions

There is a Question Sequence Number box in the question input/update page.
(This is an advanced feature so it is not visible in the default Novice mode. If the Question Sequence Number is not visible, navigate to Edit Account to change your account profile User Level from Novice to Intermediate)

For each question you want to reorder, access the question page and fill in a sequence number in the order that you want the questions to appear.
You can leave gaps in the numbers to allow for possible later additions and changes. Any questions that do not have a specific Question Sequence Number will display in the order they were entered above questions with assigned Question Sequence Numbers.
Delete Questions
- Access the question to be deleted and click the Delete question button
Advanced features
- QAAL can be used for other kinds of quizzes with right and wrong answers.
- You can provide text to be displayed depending on the student's response.
