Ethnographic Consent FormAgreement to Participate in Research
Title of Study: Social Networks Among College Students
Responsible Investigator: Mary "Mhaire" Fraser,Ph.D.
1. The research that you are asked to participate in is a question answer format using ethnographic methods. All identifying information associated with you, or asked for will be anonymously coded and kept by the researcher. No other individual or party will have access to the information. The results of the study may be published, but no individual information will be included.
2. This research is attempting to investigate how social relationships may contribute to academic persistence within the college student. As such there are no right or wrong answers. We are looking for your opinion of your experience and want you to feel free to say anything you like in answering these questions. All answers will be faithfully recorded after being anonymously coded.
3. Participation in this research is completely voluntary. No recompense is provided, and you may chose to withdraw at any time. Nothing will be lost should you chose not to participate, except our time. There are no foreseeable risks or discomforts to any participant. You may choose to discontinue your participation at any time with no retribution or punishment to you.

5. The researcher indicates agreement to include the subject in the research if appropriate and to provide results to the subject upon your request; the researcher agrees to use all information for educational purposes only. This research is to be used for and by the researcher as well for DeAnza College as a source of information about its constituents.
